We offer BlueCross BlueShield of Texas with 2 types of plans: a PPO – with two different deductible options, and a High Deductible Health Plan (HDHP), with a Health Saving Account (HSA) option. Plans include physician, hospital and surgical benefits. Medical plan enrollment also includes prescription benefits with Medco Health. Pay period deductions are pre-tax.
The Company’s Short-Term Disability Plan pays all or a portion of your income if you cannot work due to an illness or non-occupational injury for up to 90 days. Benefits are fully company paid.

Your dental and vision benefits are combined. MetLife Dental and EyeMed Vision Care are the providers. Pay period deductions are pre-tax.

Term Life Insurance and AD&D Insurance
The Company provides basic life and AD&D insurance equal to your annual base salary. You may also choose optional term life insurance for yourself, your spouse and your dependent child(ren). Optional life insurance deductions are after-tax.
This plan pays 60% of your monthly base salary up to a monthly maximum of $10,000 if your disability extends beyond 90 days. Benefits are not taxable under current federal law. You pay the full cost of this coverage and deductions are after-tax.
Long-Term Care provides cash benefits for professional care in the home, at an adult day care center, and care in an assisted living facility or nursing home. Employees, spouses and parents of employees and spouses up to and including age 80 are eligible to participate.
